TRANSFORMATION & PROGRAM RECOVERYPUBLIC HEALTH / MEDICAID

Restore Delivery Accountability in a $1.5B Medicaid/MMIS Portfolio

Troubled Medicaid/MMIS programs across New Hampshire and Montana faced chronic schedule slips and contractual damages exposure. The principal reset governance, staffing, release discipline and executive reporting within a $1.5B state-healthcare portfolio, moving the programs out of reactive escalation and placing contractual damages and SLA exposure under formal recovery management. Rationalized EPMO resources and delivery staffing to address chronic delays and execution risks.

Results:

Two state programs reset under re-established governance
$30K/day contractual exposure placed under formal recovery management

Mandate to Replace Seven-Figure EDC Costs Blocking Registry Growth

PE-backed expansion needed faster registry launches, but each new program faced a proprietary EDC system path of more than $1M to build and $250K a year to support. InteliGems led an EDC and data redesign that informed the board’s platform decision, incorporating governed data design supporting 21 CFR Part 11 controls, pharmacovigilance safety feeds and specialist workflow. New registries avoided that cost structure, freeing scarce specialist capacity for new business.

Results:

$1M build + $250K/year cost path avoided per registry
Freed 6 staff-days/month specialist capacity for new business
3d → 1d analytic prep
3+ mo → <1 mo form-change cycle

Expand a Regulated Account Beyond Traditional Offerings

A national enterprise technology-delivery account generated approximately $28M in revenue. The principal introduced higher-value testing, outsourcing, and consulting offers, secured executive sponsorship for broader mandates, and paired new revenue with the delivery capacity to execute.

Results:

$28M → $51M in year one
$80M in year two
AI STRATEGY & OPERATING MODELOWNER-OPERATOR CASE STUDY | Strategic pivot

When a Crowded Market No Longer Deserved the Capital

As meeting-intelligence competitors grew from roughly 10 to 80, the question was no longer what to ship. It was to stop funding and what to repurpose. As an owner-operator with capital at risk, the principal redirected capital, technical capacity, product development and GTM into private, governed AI for regulated enterprise work.

Results:

Hard stop
Owner capital at risk
3 AI platform generations
6–7 engineering FTE base
GOVERNANCE, CONTROLS & EVIDENCEDEFENSE / DUAL-USE

Cut False Positive Screening Noise for National Security Compliance

High-volume export-control review of scientific research was consuming scarce specialist time on false alarms while dual-use risk and compliance rules kept changing. InteliGems designed and built an air-gapped, governed first-pass AI system with guardrails that scored compliance risk, assembled audit-traceable source evidence and escalated only cases that warranted specialist judgment. Specialists retained final determination; the reported evaluation cut noise and turnaround time.

Results:

False positives 30% → 3.2%
Screening 8h → 5m per proposal
Regulatory coverage 79% → 97%
GOVERNANCE, CONTROLS & EVIDENCEPUBLIC SECTOR / JUDICIAL REVIEW

Evidence-Linked Bias Audit for Judicial Review

High-consequence quasi-judicial records require source-linked, inspectable evidence, not pattern matches. A forensic e-discovery review of a 58-page hearing-examiner ruling and related record linked claims to underlying documents, screened false positives and classified recurring bias mechanisms across seven categories. The findings were published to support the examination of viewpoint fairness, representation and potential judicial bias.

Results:

43 source-linked indicators across 7 categories of bias

Make a Seven-Figure Digital-Health License Defensible to the CFO

A proposed $1M+ license depended on roughly $300K in annual emergency-department avoidance value, leaving the investment case below the buyer’s hurdle. The advisory work rebuilt the case as four locally testable value pathways with explicit clinical, digital and financial owners. The result was a stronger buyer decision model that opened the door for deeper enterprise sales conversations.

Results:

Four buyer-testable economic pathways
2–3× expansion in modeled ROI
$1M–$5M+ potential modeled annual value

Clinical Workflow Transformation

A real-world clinical evidence organization considered a universal tablet workflow to improve data timeliness and reduce manual work. InteliGems led field research at multiple sites and found ~25% of providers were unwilling to go fully digital. Facing provider disenrollment risk, the fieldwork showed that operating value and adoption friction sat in different workflow roles. InteliGems architected a 3-channel model to protect site retention, avoid custom software builds and reclaim specialist capacity.

Results:

72 specialist days/year reclaimed
$1M+ custom IT costs avoided
100% site retention preserved

Before More Capital and Filings, Decide Which Market Is Worth Pursuing

A military-tested IV platform had 10 plausible civilian markets and no clear first buyer. The wrong beachhead risked wasted capital, retooling, and regulatory filing cost. InteliGems led customer discovery across those segments, evaluating workflows, unit economics and key opinion leader evidence and compressed validation from 90 days to 30 days. The sprint issued a No-Go on Urban EMS, prioritized large-animal field veterinary care against a $498.4M modeled TAM, and advanced a qualified buyer pipeline and master distribution terms. Leadership left with a board-ready advance-or-stop gate before further regulatory and capital commitments.

Results:

90 → 30 day validation cycle
Urban EMS: No-Go
$498.4M modeled field-veterinary TAM
Qualified buyer pipeline & master distribution terms

Turn Genomics IP Into Enterprise Offers and a Sales System

A health-risk analytics startup had validated genomics science but no repeatable B2B path. As fractional Chief Commercial Officer, the principal halted a capital-heavy consumer app direction, prioritized enterprise segments, and built the offer, pricing, contracting, and sales operating pieces needed to sell into healthcare and life insurance. Channel and enterprise pipeline discussions were advanced against a defined commercial system rather than another product feature cycle.

Results:

D2C thesis stopped
Three-tier packaging: $150K+ advisory, $25K+/mo license, $1M+/yr platform
Enterprise pipeline and channel paths advanced

A 1,100-person global account faced eroding margin, unfavorable contract terms, late delivery, QA penalties, turnover and costly execution. With $90M P&L accountability, the principal reset pricing and estimation, billability, higher-value solutions, quality controls and executive sponsorship so commercial commitments and delivery capacity moved together.

Results:

Operating margin 6% → 44% over three years
SLA penalties $2.7M → $0
